-Home Language Survey

-Kindergarten Oral Health Exam (Kindergarten only)

Only if Applicable:

-Military Connected Families Form 

-Indian Student Eligibility Certification Form 

-Medicine Release Form 

STEP #3:  

Bring copies of the following documents to school to complete the registration process:

✔ Online Verification Document

✔ Home Language Survey

✔ Special Programs Survey

✔ Verification of Age: Original Birth Certificate or Passport

✔ Complete Immunization Record

✔ Report of Health Exam (K & 1st only)

✔ Kindergarten Oral Health Assessment

✔ 2 Proofs of Residency:

✔Rental Agreement or Mortgage Statement AND

✔Utility Bill (Gas, Electric or Water)

✔ Other forms if they apply
