-Kindergarten Oral Health Exam (Kindergarten only):

K-OHA English Form: HERE

K-OHA Spanish Form: HERE

K-OHA Waiver Form in English: HERE

The K-OHA Waiver form in Spanish: HERE

Only if Applicable:

-Military Connected Families Form 

-Indian Student Eligibility Certification Form 

-Medicine Release Form 

STEP #3:  

Bring copies of the following documents to school to complete the registration process:

✔ Online Verification Document

✔ Home Language Survey

✔ Special Programs Survey

✔ Verification of Age: Original Birth Certificate or Passport

✔ Complete Immunization Record (To get a digital copy of your Digital Vaccine Record (DVR), visit https://myvaccinerecord.cdph.ca.gov/)

✔ Kindergarten Oral Health Assessment

✔ 2 Proofs of Residency: Rental Agreement or Mortgage Statement AND Utility Bill (Gas, Electric or Water)

✔ Other forms if they apply
